For Adult Family Home Providers
We partner with adult family home providers to care for the residents of their home. Our model is unique because we also serve as the medical director for the AFH. The medical director is available to support and consult with owners and staff to:
Determine whether a prospective resident is a good fit
Address state survey issues
Craft resident care policies and infectious disease control methods.
We provide primary care in the home because our experience has shown us that adult family home residents achieve better outcomes, greater comfort and increased safety when personalized healthcare services are provided in the home.
Felton Health Care is a trusted partner of the Adult Family Home Council of Washington. Currently, we serve AFHs in parts of King and Snohomish counties with a hybrid in-person and telehealth model. We are able to serve AFHs in all parts of Washington State via a telehealth only model.
For Families & Guardians
Scheduling, traveling to, and following up on any medical appointment is a huge undertaking for you and can negatively impact individuals in long-term care. So we have developed an innovative practice that brings care directly to the patient in the comfort of their residence. Instead of wrestling with a clinic or health system, you have a dedicated provider who sees patients in their home.
Providers visit each patient regularly to maintain comfort, monitor concerns, and address acute issues. If you have any questions or concerns, you can call us at any time to discuss your family member’s care.
We believe that family members are an important part of our team. Felton Health Care works with you to understand all aspects of your loved one’s health, their wishes, and your preferences. We are no more than a phone call away 24/7. We are happy to talk on the phone or schedule care conferences with family members.
