Fallan’s focus in gerontology primary care is enhanced by her ten years of experience as an RN dedicated to providing quality, direct patient care to adults of all ages in a variety of settings.

Jessica is a new Adult-Gerontology Nurse Practitioner graduate. She has nursing experience caring for those in long-term care facilities and brings compassion, respect, and patient-centered care to all patient interactions. Her DNP project focused on Dementia-specific training for Assisted Living facility care staff as they transition to memory care.

NURSE PRACTITIONER

After caring for her mom during a long illness and experiencing the impact attentive, compassionate nurses and providers had on her quality of life, she to return to school to pursue a career in nursing. Alix is especially interested in chronic and complex disease management, symptom management and end-of-life care.

Tatianna grew up working in her family’s adult family homes. She brings her first-hand knowledge of AFH operations to FHCS. She is currently studying to become a nurse practitioner through Seattle University’s DNP program.

Julia is experienced in marketing and business development of health care firms. This coupled with a background in economics, allows her to work directly with FHCS's clients to help them strengthen their commercial success.

HEALTH INFORMATION MANAGER

Merrie manages our electronic medical records, provides transcription services, and schedules appointments with clients. She assembles charts, provides chart audits, manages patient intake paperwork, organizes/file medical records, and provides monthly recaps.
